MultiCare Health System Reduces Readmissions, Length of Stay, and ED Visits for Bariatric Surgery Patients With ²ÝÝ®ÉçÇø

We are excited to share the results of an evaluation performed by a large multi-hospital system on using ²ÝÝ®ÉçÇø for bariatric surgery. .

MultiCare Health System, the largest not-for-profit, community-based, locally-owned hospital system in Washington, has been committed to enhancing patient care across its 12 hospitals. Since 2021, MultiCare has partnered with ²ÝÝ®ÉçÇø to implement digital care journeys are across numerous service lines, from orthopedics to women’s health to endoscopy and more.

One particular priority for MultiCare was improving the bariatric surgery patient journey.

Addressing Complex Challenges

Given the complexity of the bariatric surgery journey, patients often received overwhelming amounts of information, leading to unpreparedness for surgery and numerous preventable calls to the bariatric team. Additionally, limited communication post-discharge often resulted in avoidable readmissions. MultiCare aimed to standardize best practices across its bariatric programs and enhance patient education and experience.

Streamlined Digital Engagement

²ÝÝ®ÉçÇø collaborated with MultiCare’s bariatric stakeholders to customize protocols and content, allowing the care team to:

  • Engage patients via a mobile and web-enabled virtual companion
  • Standardize the patient experience across all bariatric surgery groups
  • Deliver multimedia education, reminders, and tasks
  • Track patient adherence to protocols
  • Remotely monitor post-discharge symptoms and recovery progress
  • Digitally collect Patient-Reported Outcome Measures (PROMs)
  • Automate self-care guidance via smart algorithms
  • Receive alerts and monitor dashboards to enable timely interventions

MultiCare partnered with ²ÝÝ®ÉçÇø to implement a Digital Care Journey platform that significantly improved patient outcomes and experiences:

Integrated and Efficient Care

Through ²ÝÝ®ÉçÇøâ€™s integration with Epic, using SMART on FHIR and HL7v2 feeds, patients were automatically enrolled, and providers could monitor them remotely within the Epic patient chart. This integration saved significant time for MultiCare’s clinical staff and provided a unified digital patient engagement strategy.

With the integration of ²ÝÝ®ÉçÇø and MyChart, patients can directly access their digital care plan from the MyChart patient portal. This enables MultiCare to have a unified digital patient engagement strategy.

Expanding Digital Care Journeys

Besides bariatrics, MultiCare has implemented ²ÝÝ®ÉçÇø for joint replacement, spine surgery, gynecology surgery, maternity care, colorectal surgery, urology surgery, and endoscopy procedures, with similar improvements in quality and operational outcomes. Due to their success, the bariatrics program is expanding to support patients from initial referral to surgery, thereby increasing overall bariatric surgery volumes.
