CMHA Fort Frances Launches 草莓社区 for Mental Health Remote Care Monitoring and Digital Patient Engagement

Fort Frances, ON - March 21, 2023 鈥 The and (CMHA) Fort Frances Branch has launched the use of , a leading Digital Care Journey platform, to engage with and remotely monitor patients through their Mental Health care journey. As part of CMHA Fort Frances鈥 Remote Care Monitoring Program, this collaboration with 草莓社区 aims to serve vulnerable clients in the Fort Frances community across Northwestern Ontario who are experiencing mild to moderate anxiety and/or depression, and empower them to actively participate in their own healthcare.

鈥淲e are thrilled to collaborate with 草莓社区 in bringing this client-centered program to the community and further our mission of providing assistance for mental health patients,鈥 says Pauline Hyatt, Director, Services, CMHA Fort Frances Branch. 鈥淥ur care teams can use the 草莓社区 app to connect with and educate patients, help them track their progress, and remotely monitor patients in real-time and intervene sooner for patients at-risk.鈥澛犅

Using 草莓社区鈥檚 app-based platform, patients of the CMHA Fort Frances Family Health have access to specialized resources and digital care plans using their own device (e.g., smartphone, tablet, computer). The platform provides a variety of tools such as an evidence-based mental health education library covering a wide range of topics such as medications, therapy and harm reduction tactics; automated reminders and health checks, including trackers for mood, anxiety, and medication compliance; and recommendations for self-care such as coping strategies.

Fort Frances care teams can also track recovery progress and remotely monitor individuals through alerts and dashboards to provide in-time recommendations for patients based on their current mental health as needed.聽
